You go to a ripperdoc where you can upgrade all your cyberware if you have the materials.

How long does the effect last at tier 5?

Are we talking about the Apogee Sandy? All I know it's 8 sec duration and 25 sec cooldown at Tier 5++. 30 sec at Tier 5+ maybe.

5+ has the same duration and coolown as on 5++. You can buy the 5 at level 40, and upgrade it directly to 5++ if you have the parts.

Loot is randomized, it's not a guaranteed drop on any mission.

First, go to hansen's stadium ripperdock, buy the Militech "Apogee" Sandevistan, then upgrade it as far as you can

I picked up an Apogee in Disasterpiece at Level 38 but as posted above, loot is random.

It's literally only accessible in the ripper doc inventory, pipe down.
