The story isn't changing, no, although there are some very minor additions which we know about (eg the new radio station responding to events).
1. "I just want phantom liberty, Patch 2.0 is just an inconvenient prerequisite" - just pay your 70 and play DLC.
2. "I've finished the full run 10x already. I don't want to play anymore - I'm just a slave to completionist OCD" - just pay your 70 and play DLC.
3. "I want to experience the game CDPR promised originally, and the way the books described instead of substituted, lastminute "generic mechanics"" - start new.
I should maybe point out that i've finished the game multiple times now and absolutely love the game/story, so there is that bias to consider when i say start over i guess. I'd happily replay the story over and over on a monthly basis tbh.
"i played the game multiple times before, i got to roughly 50% of the main story, and then suddenly for no reason i stopped, happened like 3-4 times"
In he's case i say, continue your save.
