Removal of cyberware has to be "confirmed" - so close the gui, let the doc do the surgery, then re-initiate cyberware purchase gui. You can't e.g. "hotswap" arm cyberware.
The easiest, least obtrusive way to fix this was to download this save editor (presumed not-virus since it was on nexusmods):

This requires no other mod libraries, mod packages, blah blah blah, its a single download, 3mb.
Unzip, run program, click "LOAD SAVE", it went directly to my saves directory. I choose a manual save folder, and a single save.dat file was inside the folder.
It took about 5s to load everything.
Click "INVENTORY" button
Look for type "[M] Cyberware" in Slot "systemReplacementCW"
Right Click, choose "Unequip"
You may want to do what I did and just unequip ALL cyberware, to make sure everything is cleared out.
Save the changes, which prompts you to choose a location (I used same file).
load game and confirm it worked.
