Not sure what the in game menu is doing to you (maybe it's asking you to go back and click through the 'rewards' and PL screens that appear before you load into game?), but PL only unlocks even for a new playthrough once you've hit and finished the main quest line job that takes you to Pacifica.

I'm playing a new playthrough now, but even my old saves loaded up and offered the new content so it should be there for you.

2.0 Patch changed everything, not the DLC. The DLC just adds more content. If you started your game under the 2.0 patch, no need to restart game.

If you load an old save from prior to 2.0, you need to see a Ripper Doc in the 2.0 version to redistribute your perks and junk. You old clothes lose their armor, etc... so see a Ripper Doc right away if you load an older save.
