The different tiers of optical camo also matter, the Legendary one makes you almost invisible, the lower tier ones less so.
There is a mod that adds full invisibillity to optical camo, so that's what you'd need if that is what you want.

All of these enable you to tweak Optical Camo way up into overpowered territory. It's up to you how far you want to go. For RPG purposes, I make Rare 20 seconds effective, with a 40 second cooldown. Epic is 25 Seconds, 35 second cooldown, and Legendary 30 sec/30 sec. Also, keep in mind that you can still be _heard_ by enemies, so you should crouchwalk to stay more or less silent.
i don't think they'll 100% spot you though if they fire back. if you're firing multiple shots into one enemy while using it, them and other enemies will just know what general direction you're in and shoot there. at least that's what i experienced when using the legendary version.
Honestly we don't know if it's a bug or intended. I'm leaning twords intended, because CDPR has released several patches since optical camo was included, and none of those patches changes how it works. So if they wanted to fix it, they had several oppertunities to do it already, but haven't.
Then again, there are plenty of other bugs, that we know are infact bugs, that also haven't been fixed yet, so maybe that's not saying much..
All i can say for sure is that it doesen't turn you invisible unless you mod the game.
Or sandevistan, where whenever you start getting a bit too much damage, you just push the I win button and kill everything.
I tend to always go pick up optical camo early on, because it's very useful in specific situations. But once you are in combat, it's not the tool for the job anymore.

I just tested and yeah it seems to be bugged, which is weird because it used to work just fine for me in 1.6 update when I started playing. Legendary one works perfectly though.
I imagine the game might not be coded to stand in front of random civilians and have them react to one's invisibility though.
So if you already get detected in other words it is useless?
The lesser rarity ones yes, but the legendary optical camo will turn you completely invisible, which will cause enemies to lose sight of you and cause them to shoot blindly in the general direction where you were last seen.
Very useful in a fight for a squishy character or if you wish to reposition.
