No, V getting shot in the head is why the process couldn't be reversed as the chip was damaged.
jackie died by fatal bullet wounds to his body. it's not like the relic automatically repairs every limb in your body then johnny comes out of nowhere and berates the user. if jackie died with the relic in his head then it would've just been a dead jackie with a broken relic in his head.
it was explained by viktor, V got insanely lucky. relic in V's head, check. non-fatal bullet wound to the head, check. relic which was inserted in V's head earlier start's repairing the bullet wound which inadvertently replaces V's memories and has what is on the relic (johnny) manifest in V's senses, check.
it's a bunch of ridiculous circumstances, like a one in a million chance kind of deal. maybe i'm missing something but this is how i see it.

Except the part where it was a prototype version, specifically designed to take over the host.
(I call it the reliquary, to differentiate between that and the standard ones Arasaka was planning to sell to people, so they could just keep copies of their loved ones)
Way I see it, the 'damage' done to the reliquary version was the main reason V survived as long as they did.
The damage probably stopped the chip doing its job *as fast* as it was supposed to.
Because frankly, guy like Saburo Arasaka does not have time to sit there, waiting for his new body. He's got things to do, can't be sat there listening to the original owner of the body moaning at him for weeks.
You sure? Helman explicitly tells us that he never got the chip to work, it was still in the trials phase, and that it working now in V's head is a "true breakthrough".
The only change to the chip since it was stolen by Yorinobu is the damage it takes in V's head, so i think we are led to conclude that the uhmm.. concussive maintenance it recived is infact what got it working.
I mean what else could account for it?
Lazy writing to be honest since we are led to believe that V is a one in a million fluke but the old man was planning on using the tech to body snatch his son.
So yeah, the Relic that we stole was a prototype chip completely different from what was intended to be sold on the market.
Fairly sure they said that they are supposed to insert the relic in a braindead person without any kind of other damage to the body, and then the relic would have time to take over and change the brain so it can transfer the conciousness from the relic to the brain. The problem is that V got shot in the head and the relic basically detected V dead and started the process but since the damage got fixed over time and V woke up ended up with the situation of an already aware conciousness getting destroyed while the broken relic tried to do it's job.
