To bad decisions Achievment bugged?

So I try to do every achoievment. have the devil and too bad decisions left. Takemura I didnt know I could save back then so gotta do a new Playthrough.
But to bad decisions is related to side quest story. I have done em all so idk why I have not gotten it. DId I make wrong choices or what?

I have the same problem. To Bad Decisions! is my last achievement left and even though I have completed it twice I still haven't got the achievement and no idea why. This game is still pretty buggy two years after release. Kinda sad.
