Remove Mods and see what mod is the one that is causing your issue cause i know that it happens when mods are involved. I haven't had the same issue but then again i don't use mods so that's why.

but there's a small chance there could be a bug. there's a known visual glitch where sometimes male genitals just...hang out....for no reason lol. Just try restarting the game.

It's a known issue that boobs can clip through clothing.

I tried out the new customization for outfits, and feet show through shoes. I haven't played in months, but it's the same shoes I wore before with no issue.
I'd imagine the chest showing through would be a similar visual bug, but can't confirm. Just wanted to add that I have a similar problem with my feet. And you know what they say about-

Somewhat serious, but it is a kind of bug nonetheless. The female clothes are all modelled after an average sized breast. If your character is the biggest size, the breast model is then bigger than the clothes and clipping may occur. Sometimes it goes away by changing clothes several times, but it seems not really predictable.

As you may change breast size at a ripperdoc, it is a possible solution, though the nude body will be different. The clothes appear the same anyway.

This only happened to me when I'd forgot to download a required mod. But you're playing vanilla, so just save and restart. Sleeping in-game has fixed bugs for me too, so try that as well.

This happens to my male V too. It can be easily fixed by removing the shoes and putting them back on.
