I remember the lighting problem. I started characters a few times at launch to compare coloring in design mode to what was displayed in game. They did a pretty good job of fixing that. I'm guessing that happened when they added the ability to update V's appearance at mirrors after starting your game.
Incidentally, I found that to be a nice story addition as well. I drastically changed my V's appearance after the failed heist in the prologue. I figured he'd be looking for any way possible to fly under the radar after that fluster cluck. Being in disguise was a logical move.
