2) yes, the three lines of code are independent so if you can crack two or all three you'll get the rewards for all of them. protip: the "buffer size" on your cyberdeck controls how many code characters you can click in the terminal, more = it's easier to get two/three of the code lines.
3) the third one is always the most profitable. the middle one is less profitable, but more profitable than the first. always prioritise the third line.

The money is set (depending on your level and which/how many of the codes you get), but the scraps vary a bit (can get 0-3 legendary, etc.), also a small (10% base?) chance to get a quickhack.
The quickhack is random, and the quality is either always green (low value terminals, these give ~half the $ which is how you know), or determined by your level (some will give legendary at level 35+, others at level 40+)
There are at least 30 quickhack terminals that can give legendary quickhacks and only require 3 intelligence, you just have to wait until level 40 (in most cases) to use them.
Generally try to get the 2nd+3rd code at least.
You can quit out of the hack before starting to re-roll the codes, but your max time goes down each time you do that.

Yes it's cumulative.
#3 - Is there a way to tell what "path" will grant the best rewards?
Take a minute to read the scrolling message for each line. And the rewards are cumulative.
But if you mean the path to actually solve the hack, it makes no difference what path you take as long as you solve it in the allotted time. However, the timer doesn't start until your first pick. So you can take as much time as need looking at the grid to see which path will work best. It gets much easier when you put a few perk points into hacking, as the game progresses.

3) The timer doesn't start until you make that first selection. You can do the patterns in any order as long as you complete the patter of level. So before starting, you can go through and review your options on getting the best path to match all of the patterns.
Some notes to remember. You can jacking out without completing a pattern will reset the options and give you a new set of patterns. Useful if you can't find a good path to get all. Only set back is that you lose a little time. But if you plan ahead before making that first click, you should be ok. I believe the minimum after jacking in and out multiple time is 6 seconds, but will increase as you improves skills and with perks. I know I have completed an 8 item path to grab all items in about 5 seconds by planning ahead.
Pattern can overlap/feed off each other as long as the characters match the code pattern. Example say Pattern 3 ended in FF. If Pattern 2 starts with FF, you can use that same FF to begin Pattern 2. So example, Pattern 1 is 1C, FF and pattern 2 is FF, 55. To get both, you simply need 1C, FF, 55. Then you will received the reward for both patterns.
Definitely agree on the first bit (2nd and 3rd should be priority).
Exception to the second bit: the Head Start perk fills in the first line for you, so you can't back out and retry; you just get the 1st line reward if you do. Maybe this changed with 1.5 or later, but I'm still in the habit of going for at least that third line.
If you go all-in on Intelligence and take the Head Start perk, then you can take the perk to reduce code length by 1 to make getting all 3 super easy.
(I'm assuming if you have 20 Intelligence you also have a not terrible cyberdeck by that time.)
I generally do terminals with low (as in ~3) Int characters, so sometimes I have to re-roll and/or accept not getting all 3.
With an 8 buffer deck + buffer extension from 19 breach protocol skill you should always be able to get 3 codes even without any perks.
also if you do this, you're going to get legendary quickhack components.
