There's no snap to target - even when you're aiming and shooting at a perfectly still target, your gun still will move due to the kick-back and maybe not hit the target entirely accurately. All stuff you gotta factor in. There's mods that lessen that effect but, so far, I've found nothing to completely eliminate it. I think they changed it to this with patch 1.5.
I can't say enough how crappy the first person shooter aspect of this game is.

Well, that may be because it's not a first person SHOOTER in the first place ...
Still just you. Upgrade the gun a couple times in an elevator and drop the difficulty if you are still having issues.
