Ultra + RTX medium.
All the evidence suggests that the game is as optimised as ♥♥♥♥♥♥♥♥. May I suggest learning from said 3 past chances and using your time to play something that is much better optimised... like Time Crisis on the Sega Saturn or Super Mario Sunshine on the Gamecube. That's what I did. Never looked back.
make sure you're actually running at max boost clocks for CPU & GPU, enable "prefer maximum performance" in nvcp for CP2077, disable all full screen optimizations for CP2077.exe.
Do you have Resizable BAR enabled? If not; you might need a Bios Update & an OS reinstall (depending on what type of partition you installed Win)
Ram running at proper clocks/timings?
You should be at 60+ FPS completely maxed out including Psycho RT. The only times you should be dropping below 60 FPS and into the high 50's is in the most resource intensive areas of the game. Maxed out without DLSS & RT OFF you should be 100 + FPS and often 120+
See videos I made of what you should expect +/- a few FPS depending on what CPU/GPU clocks you're running.
Each run starts in resource intensive area; then a less intensive area is loaded (less pedestrians/traffic)
Recording @ 1440p/60 took a small hit in FPS with RT 2-5 fps, and average 20 fps hit on the run with RT Disabled on the top end.

You also don't mention what resolution you are trying to run or what your target framerate is, which really doesn't help.

Seems like you have no idea what you're talking about. the few things mentioned improve performance yet you claim it doesn't matter, whole point of good performance is to have the system optimized to it's maximum capabilities...
the guy plays with DLSS and Medium Settings to hit 90 FPS while Same FPS is achievable on maxed out Settings and RayTracing on Psycho. you have added no advice for the guy in your rant.

also make sure you have a separate connections from The PSU to all 3 of the power inputs on the GPU (don't chain them together) if you have a mulit rail PSU then connect each to different rails.
how much power is the 3090 pulling when playing the game?
so while hits to high 50's will happen in most demanding areas, average FPS with maxed settings and Ultra RT he should expect 70-100 fps 90% of the time while playing this game @ 1440p and DLSS on Quality, now with DLSS disabled +RT is a whole other discussion

The game’s Ultra is a lot more Ultra than with other titles. It has headroom to take advantage of hardware that has yet to be released.
The unpredictability of ray tracing performance is well known. It is no coincidence that DLSS and RTX launched in the same generation GPUs!
