There's plenty of options in the game for the first, if it's the second on he's after he's playing the wrong game.
Apparently a SPRS-30 caliber to the noggin (with non-lethal module/mod, mind you) is considered a-ok, he/she is still breathing, but anything blunt to the head is still considered...lethal.
Understandable as a ♥♥♥♥-ing giant hammer swung to the face at that velocity would definitely kill, as reality ensues.
The ragdoll part when they're "unconscious" is somewhat of an issue. Sometimes they die, sometimes they don't. Sometimes their bodies explode once they're placed, and sometimes they're fine despite being on top on something/someone or leaning on objects.
For Venus in Furs, understandable. The completionist in me wants that iconic weapon, but at the same time, is it worth it to have your V get pegged harder than the bugs in the game? Depends. Me? Straight up ignored the mission all the way to Act 3. Probably in another playthrough though.
I use a 2 handed hammer and you will always get a non-lethal on the enemy if you are careful. If you do not hit the enemy more than you need to then it will be non lethal. But if you swing in 1 or more hits after the one that leaves the npc unconscious then the npc will die. So if the npc is on 1 % and you swing a normal hit, then it is non lethal...2 hits and he/she is dead.
I am running around with the equivalent of a .50 caliber sniper, headshotting people and they survive it somehow.
(on a side note, 80% of the enemies I use the suicide hack on don't die and just gets rendered unconcious, while it's a 50/50 split if an enemy dies from Overload even though it says its non-lethal).

Sorry, I know this is old but the thread I've seen on the subject. I feel the same way. (Though I don't think you should get the dildo if you say no.. that could still remain a variable quest reward lol)
The lethal damage thing happens from hitting unconscious people ... but there might be other ways to make that happen, also?
Also, I kept playing for a few more weeks and the problem solved itself weirdly. People would go into the unconscious animation every time I knocked them down. Even my lethal attacks! No joke! The dmg did a full 180 and decided to never kill another living soul again. They could fall 100ft or even get shot and have their head explode. I would turn around and they'd have all their bits and pieces back and laying on the floor. The only thing that stuck from the problem in the main post is that placing enemies still made them explode with blood unless from stealth takedowns.
(This was tested without the non lethal dmg mods of course.)
It wasn't violating it was more the witcher 3 feeling of "no, this is really gross and I didn't need to see that" that you get if you're not the target market for w3's brand of "romance" (aka bad porn).
