BUGS
====
- Mission: Don't Lose your Mind - Saving in final room can make the mission unfinishable. (My personal jack was left permanently floating disembodied in front of the terminal where you're supposed to make the final big decision for that storyline thread, and I was not able to interact with the console in order to make said decision. Ultimately had to load a save from the beginning of the mission and do the whole thing over again-- for like the 8th time, since that entire mission is plagued with bugs.)
- Mission: Don't Lose Your Mind - When you've climbed up the ladder and are trying to jump across the catwalk that has the gap in it (near where the three cars are in their own walled-off corrals, beside the electric floor), jumping over that gap can sometimes cause you to be electrocuted while in the air-- thus killing you, and forcing you to start the whole mission over.
- Everywhere: Tons of items you can't pick up (found in the world, or dropped freshly from an enemy you kill).
- Randomly, cannot target enemies with QuickHacks
- Randomly, scanning an object will not reveal what the object is until you look away and look at it again.
- Amnesis doesn't work at all.
- Datamine virtuoso doesn't work at all.
- Crit chance does not build properly. (Mine should currently be stacked at over 100%, and actually engages roughly 33% of the time.)
- Saving near enemies tends to semi-consistently allow those enemies to wander between loads. Especially if saving when stealthed, when you load, enemies will fairly consistently be in a MUCH better position to see you, or if they're far away, they tend to load facing directly at you for no reason.
- Mission to retrieve Basilisk with Panam: Inside the building, you can reach a hacking cache which will glitch through the wall and get you stuck on the inside. If you don't have strength to force open the door nor technical (or hacking, don't remember which) to open the grate, you will be permanently stuck inside.
- Enemies frequently see, and occasionally can shoot through walls (not with tech guns/abilities, but even just average low-level scags with SMGs).
- Enemies throw grenades with perfect accuracy even without being able to see you, farther than you can throw, and their grenades tend to explode the moment they're in range of you, rather than landing and carrying out their full arc and momentum like when you throw them.
- Quickhacking from stealth frequently automatically reveals you to the enemies, who rush to attack you despite having no way to have known where you are. (Particularly so with Short Circuit, more than most other quickhacks.)
- PC: No way to enter inventory/quest/map screen from the controller inputs, and how to access those screens (by any means) is never explained in-game. Running the DS4 app to allow the PC to recognise a PS4 controller for what it is will fix this, but then the game will crash at a random time, usually after between one and two hours of play.
- In the mission for the Paralez's, following the cords from the top of the house to the inside never ends that section of the quest, so it stays up through to the end of the quest line. A similar thing happens when performing the "Gig" called something like "Monster Hunt", where you can actually kill Kotaro and take his body all the way to its final location, without ever having gotten the check for "Find"ing Kotaro.
- No filters nor search function on the map to actually help you find what you're looking for. (Obvious and tedious oversight, rather than bug, but it feels like it belongs here.)
- Queen of the Highway: After Panam's meeting where Saul tells her she's right, he should have listened to her and helped get the Basilisk, she goes to a trailer to offer V a beer or lemonade, and you're supposed to talk to her-- but if you jumped behind the bar with her before she finishes her dialogue, you can't speak with her to end the quest.
- Items stashed (at least at Aldecado's camp, maybe elsewhere) disappear. (My entire stash there blinked out after one mission.)
- Often, objects you stand on will launch you off of them-- in one case, I attempted to crawl into a ground-level window, and was launched not only so far backward that it took me nearly a full minute to run full-sprint back to where I was, but it also sent me through solid obstacles. On another occasion, I was hopping a short distance down from a roof (much lower than would ever cause you damage) but because the object on the roof launched me off of it in a downward motion, the game treated it like a several-storey fall, and it killed me.
- After I modified the revolver River Ward gave me, I could no longer fire it while aiming. Had to load the game to fix.
- Quickhacks: Optics Reset seems to work very briefly, but nowhere near the 22 seconds the Epic-level version describes. Closer to 3 seconds. Meanwhile "Memory Wipe" almost never works at all, even upgraded levels of it.
- Nearly every time Whistle quick-hack would actually be useful, it's inexplicably blocked. Never an explanation, never a good reason, full RAM, plenty of capability to use more difficult Hacks (cyberpsychosis, e.g.) but not Whistle.
- Gorilla Arms state specifically that they help to force open doors, but at even a single BODY short (6/7) to force open a door, still can't with Gorilla Arms.
- Dropped bodies sometimes glitch to somewhere very far away, possibly directly into enemy Line of Sight.
- Regardless of distance, your stealth ability, or any other circumstances you attempt to create to maintain stealth: cyberpsychosis can (and during missions, almost always does) cause the enemy you hacked to INSTANTLY break your stealth, which then counts for all of their actual allies, even though the psycho'd enemy is supposed to be fighting for you now.
- When stealthed and in remote-takeover of a local network, the CAMERA YOU'RE LOOKING THROUGH can reveal you to enemies. To clarify, I don't mean that moving the camera around can alert the enemies, or that they can still find you while you're using it-- those things seem obvious. No, while the camera is focused and still, and I was sitting in a remote room closed off, I had the camera pointed in the direction of an unaware enemy walking toward it. I didn't move it at all, just watched him walk toward the camera, and as he moved to the point the camera was in his LOS, his Vision/Alert indicator started rising until he passed below the camera and moved on. I was *nowhere near* the camera, behind several walls, far away, and in the opposite direction. It's literally impossible that he could have been seeing my body.
oh yeah, and another one that isn't a bug specifically, but in the quest where you get scammed by a certain person and get transported to a scav location without your gear - if you you are not high enough level (i think it was 28) you're literally stuck in there unless you load an older save. you can't use the weapon available because your level is too low and you can't stealth takedown the enemies either.

I posted this on December 13th, today is the ONLY day (December 27th) that they actually let it get posted.
I also experienced that bug-- at least up to the point where it teleported me inside of her, but once we got out, everything seemed fine, so I forgot about it until you just mentioned it.
I am having this exact same issue. Tried loading prior versions and undressing before getting into the basilisk, didn't work. Guess I will have to avoid this mission altogether.
I made a new char. Sat in one corner and spammed the Distract Enemies quickhack for 5 hours while watching TV and it still did not work. I'd like to point out too that sometimes hackable items such as SCSMs get stuck with distract forever and can't be used ever again, and sound columns get bugged and you use Distract on them but nothing happens even though you just lost 1 RAM.
