The same with added rules/stricter rules
For example: extra attacks stacking with haste
Wheras you can get easily ten attacks in one turn from a single character, that is definitely not possible in Honour Mode as the Honour Mode rules surpress that sort of mechanic to make take away its overpowered nature. While such affect NPC enemies, it affect players most.
And bosses have legendary actions which is exclusive to enemies and bring in surprises and new challenges for new challengers.
Considering that Honour / Tactician mode are always meant to be challenging, it is wise and normal to start with the easier one first. Otherwise it is just throughout unfair due to the open world mechanic you will not get a linear challenge progression. As in: it is easy to run into obstacles which can wipe you out due to the increased challenge difficulty if you do not know about it. What is "challenging yet doable" in normal mode can be horribly hard in Honour Mode.
That much of increased difficulty you should always keep in mind.
Personally I suggest: play the game without paying the achievements any mind. Experience the game at least once without looking up what you need to do to do this or that.
You still can work with milestones savegames if you go not with Honour Mode, so you could easily reload at points and try hunting specific achievements.
Or just go with the flow and maybe you, like countless others, find the game so enjoyable that it becomes worth replaying. But first do it without spoilers.
Having a single savegame is indeed the best way to enjoy the game if it is all about not reloading just because of the dice. However, that you can achieve with discipline as well, and there are very strong reasons why you should not reload failed dice rolls whenever possible.

The game is not perfect, and unless you setup yoru Windows properly, it rarely glitches out. So even if playing Honour Mode I recommend having a backup for every game session so you can at least restore the last game session should you ever run in any of the super rare glitches. That will save your nerves and maybe some more alongside the tips you can find in the guides.

The achievement is bount to the game mode. So custom mode will not gain you those special achievements.
