False. You only need to pass the check if you did certain things.
The only thing I found in my experience that triggers the wisdom check at that point is letting the tadpole take control the first time you interact with a tadpoled body (edowin as an example) back in Act 1 or failing to resist its influence checks.
As long as you refuse the initial tadpole influence, you get a free pass with all other tadpole shenanigans.
