Do you accept the larva that leaves the character with an ugly face?
I put it on myself and the rest of my group, except Shadowheart, because it makes the character very ugly, I think I would have difficulty dating her with that face, I'm already having difficulty looking at my character, it looks too ugly

I avoided it in my first playthrough and accepted it in my second. Man was I glad to have a red skintone and Volo's eye in that playthrough, because those really mitigated downsides of the visuals.

Yes, and I immediately installed an illithid vein removal mod. I would wait until Larian has sorted out its patch issues before installing it right now.

Role play wise you would not know about those appearance changes before you try.

you also need to bypass 20 or smth wisdom check to "refuse" which can be kinda hard

TBH there's different ways of getting fly instead of going half squid.

E.g. a Draconic Sorcerer has no need for squidward's little super tadpole due to flying power at lvl 11.

Sometimes yes, sometimes no. Depends on the character I'm playing.

False. You only need to pass the check if you did certain things.

The only thing I found in my experience that triggers the wisdom check at that point is letting the tadpole take control the first time you interact with a tadpoled body (edowin as an example) back in Act 1 or failing to resist its influence checks.

As long as you refuse the initial tadpole influence, you get a free pass with all other tadpole shenanigans.

I always use the Astral Tadpole. As I also have the mod that cancels out the visual effect it puts on the characters who use it. I mean, it's a meaningless visual effect as no one else reacts to it, but I still hate having to look at it.

I don’t like the way it makes my PC look, but being able to fly is too good to turn down.
