Is there a consequence for Volo dying?
Act 3, lower city, died.

Will this make any difference in a quest? Will it change the epilogue?

Volo is both - a weave anchor and a chosen of Mystra, which are 2 different things.
As a weave anchor, should he die the weave itself will get unravelled to a degree, which will, as long as it's only volo dying and not some of the other weave anchors becoming loose as well, most likely result in heavy "weavestorms" probably wild magic surges all over the planes etc.
As a chosen of Mystra he additionally holds _some_ of Mystras power, in case she gets attacked again. It allows Mystra to regain her power through him and other chosen.
So in a sense he's a Mystraspawn, if you want to think about it that way.
Both of those are very good reasons, why Mystra herself keeps Volo so longlived.
His first notable book was released 1358 DR - 137 ingame years before the events of BG3. Assuming he was already at least in his early 30ies ther, he is at the very minimum 167 years old, though I'm not sure if his exact age is mentioned somewhere.
We however know, that Volo spent some time as a stone ornament and as a frog in Elminsters Pool, because he annoyed Elminster and others so much that it was either that or blasting it...so Elminster was the mature old mage he was and mde him spend "oh I don't know, 18 years, or 20 years or 50 years" as a frog...according to Ed Greenwood.

This is also the reason, why we can say confidently, that even if he appears to be dead ingame, in Lore he definitely is not...he's probably only spending another 50 or so years as a frog or something of that nature...^^

Considering all you just said - one would think Volo a bit more than just 'angry' and would seek 'better' options.

You lose your organic eye, but he gives you a mechanical one for a replacement. It does not negatively impact your abilities at all. Getting the eye surgery is a total net positive, even if the process looks disastrous.

Woah woah, it's not permanent disadvantage on perception checks and attack rolls in general, only against hags, which Ethel is the only one of in the entire game. It also does buff your intimidation rolls. Still way worse than Volo's eye, but not nearly as crippling as you're implying.

I am almost certain that is not how it worked before. And I played the game since the start of its 3 year Early Access period.

As I said previously, Volo is not consciously aware of his status as either an anchor, or a chosen.

Mystra actually prefers to keep him in the dark about this, because she's afraid that with how stupid Volo has proven himself to be, telling him he's endowed with godly powers might cause the plan to backfire.
