Did you? And why?
Can I choose not to become a mindflayer at the end still?

I did for the power up. I read that there's no real drawback to doing so. You can still choose not to become a mind flayer at the end.

Yes I always consume it and get everyone I have in my party all the time to, too.

Becoming a mindflayer is unrelated.

Yes I did. Because I failed the save not to and chose to go with that. It makes you a half-Illithid, which ruins your appearance but otherwise has no effect on the game except to give you some nice powers. In character, I wanted the power but knew it was not a good idea.
Then I failed the save and took it. Figured if someone has to be a monster to fight a monster, then better me than some one else. Yes you can finish the game afterwards without becoming a mindflayer.

My second playthrough, I took it to help me save the city. Next one I don't think I'm going to do any tadpoles if I can manage that.

yes, I did. it seems to unlock some different dialogue with the Emperor. plus natural Fly is so helpful.

but i probably won't do it on a later playrhough, it makes your characters look soo ugly

There is a huge drawback - you look like a meth head...it's bad enough to defer ppl including myself.

Yeah, you should use the tadpoles. They give you extra powers and will not negatively affect the ending you get. However, they do make your characters have black veins on their faces, but you can remove that with this mod: https://www.nexusmods.com/baldursgate3/mods/750

tadpole into your mouth - gives you the biggest boost in power ever possible. You can fly with Z instead of jumping, for insane distances.
You can black hole enemies off a cliff.
You can use a rank 6 buff for free.

The biggest downside is making character ugly because of the black veins.

It actually didn't change my appearance like, at all. It gave me some dark veins on the side of my head but thats it (i'm playing as the dark urge)
