Becoming a mindflayer is unrelated.
Then I failed the save and took it. Figured if someone has to be a monster to fight a monster, then better me than some one else. Yes you can finish the game afterwards without becoming a mindflayer.
My second playthrough, I took it to help me save the city. Next one I don't think I'm going to do any tadpoles if I can manage that.
but i probably won't do it on a later playrhough, it makes your characters look soo ugly

You can black hole enemies off a cliff.
You can use a rank 6 buff for free.
The biggest downside is making character ugly because of the black veins.
