Stealth Check applies when you already in stealth mode and when scan by vision cone regardless what triggers it (i.e. NPC turn around, failed Sleigh of Hands turn around, guard rota turn around, etc.) the save throw to prevent you being spotted. This is categorized by "Hiding Successfully/Failed". Stealth Check also factors into when you failed Sleigh of Hands, how often NPC turn toward you and/or runs away. (Low, quickly turn around after fail to question you; High, may turn around and/or outright scared of being a victim of theft and runs away)
But I get why the OP started this thread, because there are areas where the game is somewhat opaque on where to find this info and/or has phrasing that can be confusing.
Basically, Stealth is the skill and Stealth Check is within the skill. Stealth allows better chances at hiding... stealth check allows better chances at staying hidden.
Functionally the difference would be one adds the bonus directly to stealth, the other adds a bonus when a roll is made.
When you go into Stealth a stealth check will be made if necessary, and the numbers will be the same either way.

From my perspective, in 99% of cases, they are going to be effectively equal. I could only imagine a situation where your Stealth score is being checked directly, without a roll, that could make a difference, and as far as i'm aware, that never happens.
In D&D3.5 it could make a difference, for example, wanting to take a class that required you to have a certain number of points in Stealth. But BG3 doesn't have that. EDIT: And... iirc, its your base stealth that mattered in 3.5, bonuses from items didn't count, which is different from how the Pathfinder CRPGs handled it, but that might be a Pathfinder thing.
