You've got to reach his vault. Look down from the place where you fought him. There's a way to jump down.
Cast a 'see the invisible' to make things easier, once you're on the level below the fight area. At that point you'll just have to figure out the rest. It's more tedious than difficult. No fights or anything like that.
Raphael's bedroom has a feature that heals you and gives you the equivalent of a long rest. I missed this the first time around, but after I discovered it, it makes fighting him rather trivial. You can face him fresh as a daisy.
I did think this fight was a bit harder than the netherbrain but it still should not be that hard at level 12. You need to neutalise Raphael, (if i recall correctly I used command and then banish), while you take our the rest, then just wail on him. A few summons don't hurt as well to keep enemies distracted.
This is all pretty basic stuff really. I defended you before, but I'm starting to wonder how you made it this far. Do you have a DPS character in your party? You need someone doing at least around 50 damage a hit to get the big bads down quickly.
I would say that, at level 12, at least 3 party members should be capable of doing an absolute minimum of 50 dmg per turn. So that's 150 for the whole party.
With even a little optimization (respec'ing, etc), it's not hard to have, for example, a monk and a fighter who deal at least 100 dmg each, per turn. A longbow rogue can do about 70 on average. So right there you're looking at close to 300 for the whole party (assuming the 4th is a utility character/healer). This is without using haste or elixirs or anything beyond normal class abilities and gear bonuses.
The only thing difficult about Raphael is his AoE attacks. I clumped everyone together in a magical darkness area (I know he can see through it, but his minions can't) and could shrug off his AoE damage because I had my own healer, plus Hope. With two sources of healing, there's really no chance the fight can get out of hand.
I'm sure there's a dozen different ways to beat him faster, but that was my method. Literally zero tactical complexity. Just stand there like a moron and out-heal the damage done to my party.
Only my main tav does big melee damage but he gets at least 3 or 4 attacks per turn. Combined with Gale for AoE, Shadowheart for support and Asterion as the utility character. Got me through most big fights with relative ease.
I despised Shadowheart when I tried to make her into a damage build. Even with full Shar gear (if you send her down that path) and bonuses from the upgraded spear, and fighting in darkness, I still found her overall damage output deplorable compared to a fighter or a monk.
Again, I'm sure there's a dozen different ways to better utilize a cleric, but I haven't found them and I'm not about to use a guide. So I just make them dedicated healers and damage sponges. Seems to work adequately.
Once the portal is open, the nether brain actually dies very quickly. Aside from those time bombs that destroy the platforms, it doesn't really have much defensive capabilities.
