Of what i've seen you can side with the Emperor till the end then betray him, either by telling to Tentacle off, and wich point he'll be like "♥♥♥♥ you all, i'll join back with the Brain and kill you all" (strange i know)
Or you can stab him in the stomach last minute and free Orphyes
You'll need to convince Orphyes tht you're on his side, if Leazel rating of you is high enough or you romanced her, it should be fine, but i ain't 100% sure of it.
The Bit with Karlach is, that the Emperor will either have to devour the Prince's brain in order to gain his ANti-Illithid power, OR one of the character HAS to turn into a Mindflayer and eat the brain (can also choose to not do it, wich leads to the Emperor been butt hurt and deflecting).
If Karlac is in the Party, she will volunteer to become a Mindflayer, since she knows that she's doomed, cause her Engine cannot be fixed while she's on our Plane, and sicne she is adament to NOT return into Avernus, either she dies when everything is done, turning into ashes, Or she Goes back to Avernus Or she turns into a Mindflayer and stays "alive", at least for a while before her soul and personality fades away leaving Only the Illithid...(the game narrates that "OR Maybe you'll be a unique Illithid like the Emperor?, retaining your memories and personality? who knows?")
SPOILER WARNING: Is the Lae'Zel check the one in the camp in Act 3 when the Githyanki matriarch (forgot her name) shows up and asks Lae/zel to kill orpheus? I think you get a difficulty:30 persuasion roll for Lae-zel.
I actually left my game saved at that very moment last night cause it was late and I said screw this after I failed the roll, this seems like an impactful choice, potentially being at a cross roads with lae-Zel. I just wanna finish the game TBH at this point. I stopped caring about the story. I havent scummed dialoge rolls in my game but I might for this one, should I care? I just dont wanna lose Lae Zel at this point of the game and have to learn a new character.
However, he does so with the final wish that you kill him once the task is complete, meaning that he is a far more preferable option to allowing the Emperor to pursue his machinations, or to surrender your own character's soul and risk becoming a monster in the future.
