If you are missing constantly with attack rolls then you are failing to get high enough numbers to get past their armour, which is another generalised term for describing how hard you are to hit. If you are failing with magic attacks then they could be failed attack rolls or the enemy is easily making saves against your spells, different spells behave differently.
The bigger the difference in level between you and your opponent the harder it is going to be to hit them successfully no matter what the method. Right mouse click on the guy and choose Examine then take a look at his AC (armour class) and resistance, ability scores, etc.
Also try to think outside the box. Are their environmental elements you could exploit? Are there other strong NPCs around you could convince to help you?

I always rush buying +1 weapon as soon as possible bumping the percentage to 75%. Since I’m playing Rogue, having advantage + sneak attack bump that odds to 90% odds.
You must actively debuffing enemy lest you have to face their DC raw. You’ll likely to hit a distracted enemy than you do a focused one.
Every 1/20 is kritical mis in fight so 5% of will mis automatically and if your skill is 60% to hit… You will miss allmost half of your attacks so you should increase your ods by getting advantages that gives you two dies!
At the start, yes. Your job is increasing your success chance through character development, spells and items.
As for that specific fight, you generally can't brute force it unless you are level 5 or higher. You need to be more tactical.
Spells like Sleep, Hold Person, Guiding Bolts, and other powers that give your opponent disadvantage or you advantage are very helpful.
In most games I have played that show a chance to hit of 60% it is pretty much a guaranteed miss. No matter how accurate the CTH is supposed to be I would double the amount it is below 100% and you will be closer to the actual results. So for 60% it is 40% below 100% so make that 80% and you get the real result of 20%. This is not based on any proven game mechanic but if you try it you will see for yourself how much more accurate it is than what is displayed. RNG sucks. Dice suck.
Some games give you a CTH which does not take into account almost every defensive ability the enemy has like dodge or evade. I have no clue how this one works to be quite honest but I have found that the more important it is that you land the hit, the bigger the disaster if you miss, or if the game senses it can just irritate the hell out of you by missing then it will do so.
