Though officially speaking, demons have no legitimately recognized hierarchy. They are the physical embodiment of Chaotic Evil, and as such, their leaders only maintain their positions for as long as they have the ability to defend them.

Asmodeus very rarely gets involved because it helps keep him in power.
Tiamat is hiding/trapped there, and depending on player choices may in fact get involved and take over, but nothing canon.
The gods and other Celestials generally don't get involved because it's evil fighting evil, Asmodeus won the legal case, and without the Devils the Demons would overrun everything.
The angel Zariel didn't like this, got involved and it went very badly for her. She is still currently an Archdevil and in charge as of BG3. The previous ruler, Bel, is presumably either dead or still pissed.
The hells in general, yeah. Avernus specifically, not always but definitely now.
