Heavy Load Damage
This has probably already been brought up, but I'll post just in case it hasn't.

I think carrying too much should either injure you or give you a really high chance of spraining/breaking your ankle or hurting your back. When I say too much, I don't mean a few extra Lbs. I'm talking about carrying significantly more than you should be able to; something along the lines of 2+ times your characters initial carry capacity. As you carry more the chance of injury would go up.

The injury would decrease your carry weight below normal, reduce your accuracy, reduce your damage with melee weapons, and cause you to be in pain. Painkillers could, obviously, be used to negate a portion of these penalties.

Crutches and splints could be added as a found item or a crafted item. Neither would take carpentry skill, and both could be crafted using a planks, duct tape, and towels (for padding). Using either one would decrease the severity of the injury and reduce the penalties it gives you.

If all of that sounds like a little too much, then maybe make it so your character can't move if they're carrying 3+ times their capacity. The whole reason for this post is because I shouldn't be able to stand with an inventory that's 240/16 and nothing but logs. The only reason I did that was to see if there was already a chance of hurting implemented, but all that happened was my inventory bugging out and not letting me directly drop things (I had to move them to my hiking bag and then to the container/ground).

I could've sworn it already does this. A few weeks ago when I was playing I was making my way home from a run and was overloaded. By the time I got back to my house my character was experiencing pain with no injuries.

Well earlier I was walking around with my inventory at 240/16. Left it like that while walking around for a couple in game hours, but nothing happened. Maybe the amount doesn't matter but the duration of being overloaded does? *shrug*

I know in pre-steam builds walking whilst being heavily overencumbered like this caused damage like Death says, and it was quite severe and quick to happen if I recall. Since playing the 2.9 builds I've always avoided being so encumbered... Will play around with this a little tomorrow and see if I can find out more :)

I'm sure I went to sleep with my weight at like 250+ and suffered no negatives except the out of breath constantly and slow movement. I remember in old versions where you got injured from it though.

Is "too heavy" determined by weight or number of items in your pack?

It's sort of by weight, although bulkiness plays a part too. Each item has an encumberance value based on it's weight and size. Planks for instance have a high value, whilst IRL they're not usually very heavy, they're usually long and awkward to carry. Putting stuff in equpped bags offsets this value by giving an effective weight reduction.
