Needs a rework. I propose that you lose endurance quicker and cough when exhausted, while removing the random 30 second coughs from not inhaling a cig every 2 hours. Keep the unhappiness and stress the same. Maybe making it a +3 or +4.

People need to stop complaining about people giving feedback on a game that they play. While we're at it, why don't we balance all of the negative traits so they're never worth taking? That will make the game so much better!
I assume that the point of having negative traits is to allow players to choose trade-offs; there are downsides to the negative trait, which allows you to choose positive traits that you otherwise couldn't afford. This only makes sense if the relative cost of the negative and positive traits are reasonably balanced, leading to interesting choices.
Smoker was not well balanced in B41; it was effectively "free" 4 points, and so practically everyone took it. There were two changes in B42: smoker was reduced to +2 points, and cigs became MUCH harder to find. This seemed balanced... stress was an issue, cigs were hard to find, and you would sometimes cough after smoking... but you had agency over the timing of the coughs, because they were triggered by smoking, so you could put off taking a smoke break for when you were safe. This seems like a reasonable tradeoff for being able to, for example, take cat's eyes for improved night vision, which is the same cost. Prior to the recent change, I was on the fence about whether to take smoker or not; I kept it partly because I liked the puzzle of hoarding cigs, even though I wasn't sure it was worth the 2 points.
Now comes the recent change. I get the intent behind this, it adds an interesting mechanic to smoker, but I don't think it makes sense at only 2 points any more. If the intent is for this to be a meme build trait where people only pick it as a way to play hard mode, fair enough. My 2c is that it no longer feels balanced. I think the balance was about right before the most recent change. That's my take anyway.
That's under my assumptions about what you're trying to do with traits, though. If negative traits are ACTUALLY meant to be "hard mode", and are really a way of adding interesting difficulty sliders to the game, that's a whole other design philosophy. If so, then smoker is fine as-is: I'd only take it as it currently stands if I was bored and wanted to make the game harder for myself. That's a valid game design choice too... up to the devs how they want to run it!
I thought the negative aspect was constantly having to manage stress and a surplus of cigrits.
Now we have a constant cough as well? It's not about ReAliSm, but positives and negatives.
make smokers (just players with the trait not zoms/other player) almost 'deaf' to their smoker cough just like they are IRL lol
Unless you're increasing day length, that's still every 4 to 12 ½ minutes of playtime, RNG dependent.
make it some kind of action completion generic thing so it can be used for spicy food and raw potent alochol consumption
Smoker puff a cigarette doesnt cough as a perk to being a smoker.
Heavy Smoker cough all the time. at every possible opportunity and randomly thats u get payed double for that.
Nobody drinking 1/4 bottle of bourbon few swigs with out OUEHH moment
or vodka. Its just a cough with a differn't text liine.
Nobody go to zombie infront of zombie eat a Spicy Chilli, or Tobasco infront of a zombie, or a Soy Sauce too much soya sauce, or even just a luxury Toffee bit sweet randomly cause a cough.zombie will notice if you eat that infront of it. If you ate a spicy burrito. With exception of Iron Gut.
negatives for the smoking trait were already there, but immersive and not overly annoying
-constantly looking for cigarettes and lighters
-extra inventory management
-nightmares if you forget to smoke before sleep
-frequent agitated debuff
but becoming a zombie magnet is annoying, all for -2 trait points
