Usually this looks like this:
KoS forbidden in most areas, allowed only in (example) WestPoint, or when somebody raiding your shelter, or when you raiding somebody. (Servers specipicate what count as shelter).
Also if you are on pvp mode (skull on), you are viable kos target.
Before attacking somebody, you need to warn the other player, you have to use local chat. Warning should happen X seconds before the attack.
As a veteran, I can say these rules are important. If there are no rules, the server shortly goes deserted.
Should just make it so that cities have pvp enabled, that way everyone knows that if you enter then you could potentially get killed by more than just zombies.
