You are literally the only character so that's an odd thing to say, did you pick this up from the Forever Winter or something?
Cultural might not be the right word for it, but I'll try to explain to clarify.
The Literal meaning of main character is just that- The one the story revolves around. They are the primary focus of the experience; And in PZ, that is true, The player character is the Literal 'Main Character' since there is no other character.
But, The Cultural meaning of main character is the one who has plot armor and always manages to get away even if they get roughed up, The one who has unexpected windfalls save their ass, the one who lives a long relatively happy life and dies at the end of old age married to their love interest.
To compare it to other media; Every character in a story is the main character of their own life. Nevertheless, There is undeniably one Main Character whose events sway everything. Side characters die all the time- But never the main character.
In PZ, We are the side characters. The story we are telling through our actions is the one of how we died- Because we weren't the Main Character, We don't have plot armor, Nothing is going to save us from our abysmal fate. There is no heroes sword, There is no mage to pop out of a random crystal ball, We're not going to find a fully furnished fully stocked bunker that will let us live for decades without hardship... We're just an average joe, who while the main character of their own life, is not really anyone particularly special.

Because there are ways to avoid the infection. the more layers of clothing you have the lower chance of getting a bite. also, dying isn't the end, you can make a new char and reclaim all your stuff.
Ripley died to kill the alien queen and save humanity. In zomboid you die after growing a few carrots. You aren't the main character anymore than the survivor zombies you bump into now and again are. You're another nobody amongst the thousands of nobodies who dies.
I usually play on infection through bites only, with other difficulty rebalances to compensate for it.
The RNG of scratches and lacerations just feels bad to me.
If you turn off infection it becomes almost impossible to die. Which is really boring IMO.
