This is not fully correct.
Wearing clothing that is dirty and bloody will also increase the chance for injuries to become infected.
IIRC there is big plans for a first aid overhaul in the next build, so this information would be very relevant next update, where preventing infected injuries is more substantial to survival, than a mere annoyance to the player.

That's only if you have fear of blood trait. Which increases your anxiety level when you're covered in bloody clothing, which will then start adding depression.
If you have anxiety in general, it will add depression around 50%.
Just normal wound infection? I suppose I've never really noticed.
Why do we have to read magazines when we dont smoke?
I don't blame anyone for not noticing this, in general, most players will practice good hygiene and self maintenance.
I only noticed when I started doing harder challenges that let me far less or essential no time at all to do either.
Besides not smoking as smoker, and being covered in blood with Fear of Blood, you can accumulate stress if your character is constantly hearing zombies beating on doors or windows nearby.
