Perhaps you get damage when you sprint against a zombie and it infinitely repeated it within a half second?
So instant death turned off, you mean you turned off the setting where you instantly die if you get infected right?
Because if so, you actually most likely enabled it. Because on default it is turned off, so if you changed that from default you enabled it:) ->because as you describe it seems more like a got infected and instantly died.
Or you mean the drag down kill? Where zombies instantly drag you down and then you die? (Cuz how many zombies is needed also depends on your nimble skills, i once survived 5 zombies trying to drag a char of mine down)
Cuz the details are i have to say not really clear. How were you fighting them?

Since yuo did not take a screenshot;) Also the easiest option to check out what happened, is to try to get into a similar situation and test it. It is why i keep backups. There are many things that players do not know about and discover themselves or via the forum or guides.

X being me. I sprinted into them to provoke a fall to escape as usual, as the one diagonally behind got me cause I got low fps there due to old blood stains from nearby ~4k kills (already reduced blood).
Yes. Curing infection with a mod is known to cause occasionally strange behaviour. So most likely that thingie about the infection level becoming suddenly 100% happened. I don't know and understand that system there was some stuff explained about it, and if i understood well in most cases when you "cure infection" via mods, debug etc, some kind of a bug might occur making you not cured fully but stuck on a certain level, and being hit can get that level to 100% instantly and you die. ->this is my best bet. If i did not misunderstand that whole discussion.
The other option would be instant death settings for infection, but since you already cured one that is not likely.
The dragdown as you mentioned is turned off so that is not a cause.
Sadly i would need to replicate the issue, but i dont wish to mod my game further(i just have pillows rs). What i can suggest to you is to try and replicate the issue that way you might figure something out.
