The longer you wear bloody clothes and the bloodier they get, the more stressed your character becomes.
And I don’t want to go on about it
But….
When you have a bandage on, it ticks on a clock, and decides at intervals whether it will be infected (normal infection), and transitions into a dirty state. We know that a fast healer has less tick intervals and good first aid also reduces the number of these tick intervals. While slow healer greatly increases them. Most people believe that this is all that happens on tick, but it’s entirely possible other things happen. Like a check for zombification chance. And we know that normal wound checks for hygiene aswell. A dirty person has more infections, makes sense. If you were completely filthy you can be sure every wound will get infected. But as for zombification - is it included - is there a memory of the wounds origin that is then referenced to decide whether to reroll for zombify chance - this is the great unknown.
I just play with total immunity. Not a fan of the infection mechanic.
Blood and Saliva = 7% scratch, 25% laceration, 100% bite to receive the Knox Infection which will kill you.
Saliva only = 0% scratch, 0% laceration, 100% bite to receive the Knox Infection.
Dirt and Blood on your clothes has no impact upon whether or not you'll get a wound infection, or any real effect whatsoever at this time. You could be in 100% dirty and blood soaked clothes and as long as you keep the bandage fresh, it won't get infected. Leaving a dirty bandage on for any length of time is what gives the risk of getting a wound infection. Further, Blood on your clothes cannot infect the player with the Knox Infection.

Blood on your clothes will cause you to gradually accrue stress over time. The more blood, the faster you get stressed. Blood on walls/floors around you also will stress you out. Additionally, as already noted, Performing any medical actions on yourself will make you panic- And in multiplayer you straight up cannot perform a medical action on other players.
To make one important note, The word 'Infected' on the health panel is a WOUND infection, Not the Knox infection. You can get wound infections and be perfectly fine. Meanwhile, the game never explicitly tells you when you've contracted the knox virus except when it's a 'Bitten' wound; And even then the game doesn't tell you, it's just game knowledge to know that a bite is a 100% chance of infection.
The knox infection presents with a host of minor moodles, but as said it never outright tells you.. You'll have some pain, some stress, some panic, some rising sickness, and eventual death- But all of these are things that can happen from other sources.
