I don't know what just happenned with the thin skin trait

so i just started a new character with the thin skin trait and was just vibing, i played for a bit more than an hour and while i was eating some food the message "Your Thin Skin gave you an additional wound on your ____" only it was in every single part of my body and i instantly died. Is this a bug with a mod i'm using or was i just unlucky?
