Clarification on "underweight" trait
I usually use underweight to get some points as it's easy to regain the weight and lose the trait.

However, it gives a debuff to fitness.

Do I get the "fitness" back when I get above 75 weight, or is that a permanent loss that can only be gained back slowly ingame?

Permanent loss. You lose the other debuffs but don't get the 1 Fitness back when the trait goes away. It's still free points because Underweight and Fit is a net positive.

Correct. But you last more with little food in winter, if needed.

ohh intteresting, always thought they were good traits that you can overcome, didnt know they made a permanent loss to fit.

If you tune up fitness exp / get the books mod, it is manageable. Otherwise expect months, if not years, of exercising to get fit.

In my view the grind is a complete overkill, in 6 months you can get very fit IRL, especially if you are exercising all day.

So if i take the unfit trait what does that mean exactly? Cause i took very underweight and unfit cause it comes with it anyway. You can eat your way out of very under weight but i thought my fitness was improving. I dont seem to be having any problems running around.

You will get exhausted much earlier, with severe debuffs. Do a lot of squats every day to improve it.

I dunno how long i have survived. I have eaten so as to be just under weight and i got one half bar of fitness. I dont feel particularly restricted by it tbh.

Yeah exercise is something that needs adjustment. I notice I can gain fitness experience by all sorts of normal activities, albeit in small amounts, but the gain from exercise is so tiny itself there's no point. Why bother dedicated activity when it's no better than what you gain merely by existing, AND messes you up physically until you've been doing it for a while? At an absolute minimum the "exercise while wearing a backpack" mod needs to be core gameplay, because there's no reason to make a tedious activity even more tedious by forcing you to adjust your inventory every time you do it.
