Do I need to unsubscribe all mods from steam workshop to be sure no mods are affecting game?
Steam Library > Right click Project Zomboid > Properties > Local Files > Verify Local Files
If it still fails, can you start a new game with the issue and then share your console.txt file found in C:\Users\YourUserName\Zomboid?
You can use any file sharing website of your choice or file.io and pastebin.com for example.

In the load game screen it says there's no mods and I disabled all mods in the main screen too, so it's odd for any mods to load. Is it worth trying unsubscribing to mods in steam workshop?
This should wipe and custom and mod settings. You could also just delete the "sounds" file in that folder as perhaps there are some custom settings applied there that would disable the sound, though deleting the Zomboid folder is the more surefire option.
I unsubscribed from all mods in steam workshop just to make sure but still no luck, I think this started happening a while back 41.4 or 41.5 should've reported it back then

Honestly its not a big issue I'm always on the lookout for queasy moodlet and there's plenty of time to deal with it. If there's other things u want me to try let me know.
If u can see them flying around but not hear them give above suggestions a go, fixed it for me.
