Depression is gained by either not smoking as a character with the smoker trait or having high boredom.
With Boredom that's much easier to fix
Go Outside
No, really. Going outside drops boredom. I often do my reading on rooftops outside and I never get bored. Another way to not get bored is with panic. I you have Claustrophobia and shove yourself into a tiny closet and do some reading you'll be panicked but not bored.
As for why they're in the game? Dude, psychotic breaks and deep depression are staples of zombie apocalypse settings. Ya know, unless the writer is deliberately going goofy with it. But this isn't 'Zombieland', Project Zomboid is serious.
Technically, boredom leads to unhappy which leads to depression.
When your character becomes unhappy, actions are performed slower. So yes, there are actual consequences in the game.
