Are you moving, or feet planted? Are you panicked? Lot of things go into it. But the two biggest ones are panicked, and people moving while they're trying to shoot.
Enable the aim outline in the options, and watch how the outline color changes as you stand still while aiming at a target, and watch how it changes when you start moving. Stop, Aim, Fire, Move- Do not fire on the move or you're going to get exactly what you're running into.
I miss maybe 1/10 with a pistol at point blank with 1 aim skill when fairly panicked. By aim 4, I can fire a pistol at targets up to half a screen away and still hit them 80-90% of the time.
