As to what is causing it, your health dropping slowly over time sounds like you've either carrying to much or have caught some kind of illness. Carrying too much can quickly drain your health.
- what injury you have? scratch, laceration or bitten wound? these wound you need to apply bandage to stop bleeding and even after healed you'll have some chance to get zombie infection which can cause sickness and no cure in vanilla game if these wound caused by zombies
- if you have no wound, is your character sick? you can get sick from eat uncooked foods, eat spoiled foods, drink tainted water, or spend some time near corpses and inhale too much miasma
- if no wound or no sickness, do your character carry too much weight? carry very heavy load can cause damage to your body aka reduce your health bar
"minor damage" just tell you that your health bar is slightly reduced, sometimes it just health drop and doesn't need addition care. Eat some food until you gain food buff and your health bar will start regen (even without food buff it can regen but with very slow rate)
