I've loaded several worlds of CDDA which i believe have rare loot and i only found 1 in a cardboard box (the smaller version). It was in Mauldragh just above the Bookshop in the next room there were 2 boxes and 1 of them spawned it for me.
Couldn't find them in:
- Large Wood boxes
- Large Cardboard boxes
- Medicine Cabinet
- Bathroom Counters
- Any Wardrobe variants
If anyone else has found them in any of the above please let me know. This was done in 41.53.

And to be honest, I don't have a single needle anywhere in my own house... it think.
2 in a factory, 1 in a random house
They are very rare in 41.53
Way too rare...
A needle is pretty crucial for survival if you have a deep wound, I'd say.
Edit: I suppose you can use surgery needles for stitching yourself up though, now that I think about it.
I think game balance is important, but honestly if we want to be realistic here, you'd want a totally separate class of needles for leather anyway. My suggestion is to make the player character struggle to use a normal needle for leather patches. I don't know if anyone has done basic clothing modification, but patches like we do in PZ would be time consuming and difficult even for the denim and cloth patches, never mind leather.
Make it much more time consuming to do with a normal needle while doing leatherwork, and have the character injure their fingers while performing tailoring and eventually make them unable to continue to practice if their hands are too injured. Less so while working with fabric, but the player should have more of a chance to injure themselves at a lower level.of tailoring.
No idea when Project Zomboid is taking place but we definitely had needles at our house in the 90s but almost never used them. If it's in the 90s or further back it'd make more sense.
IRL: I have needles and i still use them. When a button falls off my shirt or coat, i wont buy a new one, i sew it back. Hell if i recall well in the late 90-s (ok it is Middle-Eastern Europe) we had one or two sewing classes even. To learn some kind of basic sewing, was mandatory for everyone.
In game: It seems that there is a huge distinction now between the "Warehouses". Not sure what their category types can be beside warehouse and factory, but it seems that certain types have certain items. You can find antique oven in one and none in the other, for instance. Pretty sure it is the same with needles, not to mention sewing kits are even a more rare find.
