Not quite the same but people who enjoy PZ would probably also enjoy Darkwood and NEO Scavenger.

If we're talking survival + quality: Don't Starve. But you've probably heard from that. Still... KLEI makes really good and solid games.

3D "ripoff" of PZ would be Scum? Not sure how the game is now. Haven't played that in ages. But yeah... it's basically the same game without the hordes, although you can edit some files to spawn in more. But not PZ levels of more.

Seconding Cataclysm: Dark Days Ahead. It's free and open source, too. Just be prepared for a steep learning curve due to the keyboard-based controls (like... the WHOLE keyboard, including upper and lower case letters).

Yes to CDDA. I've been playing that all evening. Usually I barely get out of my starting position before I get torn to pieces but tonight for some reason I've been able to run around like a maniac killing everything with a golf club. I might actually live long enough to die of dehydration.
Just to warn you though, CDDA's steep learning curve makes PZ look like a gentle slope.

Just a word of warning for y'all: Darkwood is a fantastic game...it's also the scariest game I think I've ever played. It is genuinely terrifying, there is something done so right about it. A horror reviewer who is a literal adult that I know, felt that he couldn't beat that game. I didn't play it much; got sidetracked and lost the horror mood, but ye.

Players who like this game usually likes Exanima.
Both games are hardcore, scary, slow paced and so on. Exanima is NOT sandbox though.
Both games at release will have LAN as well, which wont force you to play online like other stupid companies do. This is for me a heaven sent as I live out in the forest with my family and my ping and such always gets super high.
If any games are wort supporting, these two are it.

I dunno, maybe consider the Sims 1, that game is pretty similar. Or for a more serious answer, maybe you'll like game called Dead State, which is a turned based zombie survival game where you fortify in a school and visit locations around you to scavenge supplies and meet survivors etc. It's pretty good...but obviously the gameplay won't be anything similar to Project Zomboid, considering it's turn based lol.

It is the only game in this niche

CDDA seems to be the closest i know of, just more hardcore and complex...
But the UI....mmm...nope...i just cant deal with it nowadays

There is Dead Matter...promising....
But ive always been suspicious of too ambitious EA indies in 3D...

Dead matter seems promising indeed. It has improved a lot if you watch new videos for it.

The Long Dark is a great survival game, and trust me, a bear is just as terrifying as horde when you get caught out with one. Games terrifying and beautiful, and I really hope PZ takes some notes from it's food system. Dying from food poisoning so often is not fun or realistic, but the food in PZ is so bountiful that you never need to worry about it.
nope, this games one of a kind. As others have said, CATA DDA might be the closest. But this game captures the feeling of being in a zombie apocalypse like none other. I always keep coming back. I tried Survive the Nights but it felt too janky, unpolished, still not as immersive

Once upon a time there was a game called The Dead Linger in development. It was a first person zombie survival, very similar mechanics to PZ. Showed huge promise, went through 3 different engines and finally the ♥♥♥♥♥♥♥ ♥♥♥♥♥♥♥♥ scrapped the whole game and took it down from steam. They then released a statement pretty much blaming their backers and they changed the company name and ♥♥♥♥♥♥ off. The End.
