Accuracy is determined by time spent still, panic, and skill level.
Its sooooooooo good
Second, you need to stand still for a second or two to aim properly. If you're trying to shoot on the move your accuracy tanks and you likely won't hit anything. And this is where those highlight colors come into play. As stated by someone above green means you're good and your hit should land and possibly kill your target and red means you are more likely to miss.

I'll assume your only experience with firearms is seeing them in movies or video games, where any nuance about their operation is conveniently left off screen, and its just as simple as pointing and clicking.
You should be thankful at lvl 0 aiming, that you don't drop your weapon or accidentally end up shooting yourself on occasion. Because you can find lots of videos on youtube of people who don't know how to use guns, firing in perfectly safe and ideal conditions, and still hurt themselves.
Add in panic, malnutrition, or exhaustion; like what your character in zomboid can experience, and people would make more frequent and even worse mistakes.
Even shooting a weapon that is in poor condition, be very thankful it just jams frequently. Because a damaged gun could take some of your fingers off and shred half your face when the bullet comes out the side of the weapon instead of the front.
