Clothing and dirtiness
I've watched my char put on clean clothing and read a book for 4 hours sitting in a clean room to have that clothing now say (dirty).

I take it this doesn't scale to timescale of player?
Anyone know a way to manually change it?

By default its sets as "Normal" but i found it way to fast
I using Low speed, its still getting dirty, but atleast not right away from killing 1 zombie

Depends on other things such as body temperature, wetness, and perspiration. I've had a character wearing fresh clothes, not do much for a day and still keep them clean by the end of the day.

Nah, you get dirty so easily. I'm doing a no melee combat run at the moment, and what I notice is that water is enough to clean dirty clothes, it is soap you need for blood. I don't get blood on my char, so I just need water. Yes, still going to get dirty very quickly.

Can this value be changed after game start?

I dont think so.
You could change server setting in old version even if you play solo online hosting for yourself only, you could change any setting in midgame, but i dont think you can in solo, especially in b41

what do dirty/bloodied clothes do anyways? do they make it more likely to get an infection or something?

i think they affecting general mood, like waking up during night in panic etc, not sure, but it happens less when im cleaning my clothes regularly, and perhaps affecting depression
but again, im not 100% sure

I think, if you have dirty clothes and get a skin wound, it has a better chance of being infected. The same for if you are covered in blood and grime. You can wash up at a water source.
