IRL; the alternator of a vehicle will only work if the battery works, and depleting rechargeable batteries fully is a good way to kill them. This isn't realized very well ingame though, I agree, but vehicles are new to the game in general so I wouldn't be surprised if this was looked at sooner than later.
Everytime you start a vehicle the battery drains around 2-5%. If you are constantly starting the engine and turning it off shortly after you'll drain the battery pretty quick. I did this quite frequently e.g. drive a vehicle a block, turn it off to decrease noise while filling the trunk, repeat.
If the battery is completely drained I don't believe you can charge them.
It seemed to me that vehicles I found later in the game, that is vehicles that were rendered/spawned during the winter had a lower battery charge.
But now we have car battery chargers.
