As for exotic items, again, pinnacle activities will provide those as well at higher levels.
You can focus your exotic engrams with Rahool at the Tower but for a cost. Focusing them will guarantee a slightly better roll of perks usually no less than a total of 64/65 which when fully upgraded becomes 86/87. I got a pair of Orpheus Rig's at 68 which comes out to 90 fully upgraded.

For the best roll using an Exotic Cipher with an Exotic Engram supposedly guarantees the highest chance of a good roll but I've never taken the risk as I like to use the Ciphers to unlock the additional mod slot once you fully upgrade any Exotic armor.
Your post lacks specifics of what exactly you mean by "skill issue" so I could only presume that you're meaning gaining higher level items. Apologies in advance if I missed the mark.
Wrong, I consistently can get 60-61-62 exotics from focusing exotics. Not worth it. There is no guarantee.
