Crota's End lamp encounter bugged for anyone else?

Our group has now 3 times in a row had our 3rd podium not trigger the timer's extension and as a result wiping us with no recourse, as the rest of the lamps stay unpowered. We have made sure on the third try that it's not simply negligence missing a podium, has this happened for anyone else?

If you're depositing, make sure its not with a couple seconds left. Make sure someone picks it up as soon as possible, too. That's all there is to it.

I understand the mechanics, I've run the raid at least a dozen times at this point. I'm saying we preserved the chalice at every podium we come across, and on a specific podium spawn we would preserve (on one attempt we had 16 seconds left on the timer) and reclaim and it never registered. We had not missed any podiums. I'm not asking for help with mechanics, I'm asking if other people have had this issue happen to them as well.
