How to submit a problem ticket

First off, how do I submit a ticket to customer service? All I can find on the "help" website are articles that do not have a way of fixing my issue.

secondly, my issue is that when I try to accept the EULA, it won't let me. It starts filling the bar underneath, gets about 15% of the way then resets. I have played the game a long time ago, but now it is literally unplayable.
