Is that when theres no borders, or is that where I can see around it a little bit?
Just looked at my settings, Yes. It is in Full Screen.
Literally not even what OP was asking but ok..
That's fine, neither am i apart from a few things here and there.
Try changing to borderless and back to full screen incase that might do something.

Last thing i could recommend in case you're using NVIDIA, try rerolling your driver update back to a previous one since the current one is causing issues.
when you say its not coming back into focus are you meaning that if you alt+tab out, the screen stays black when try to alt+tab back in?
Or do you mean that if you alt+tab back in, another window for a different open application sits on top of the game window or the Task Bar remains in view even though you are in fullscreen? This would technically be what is known as "stealing focus".
If it is the first thing, then possibly a display driver problem. when a game is run in fullscreen, the desktop is no longer rendered as the GPU is exclusively rendering the game window only when it is in focus. this is why it takes a second if you alt+tab out before your desktop is visible and vice-versa. I would try updating or re-installing the latest driver for your GPU or rolling back to a previous version (Display Driver Uninstaller is a handy tool if you clean driver uninstalls, but not necessarily required). You could also try setting the game to run in Borderless. This is essentially fullscreen, but the desktop is still being rendered since the game is just running in a fullscreen window with the ribbon removed.
