you can purchase an item in the Eververse with bright dust that guarantees one will drop from a major enemy when consumed, but they will run you 200 bright dust a pop.
your other options are buying them from banshee or ada-1 or get them from the season pass as you rank up, if you bought the season.
the other way of getting them is doing the witchqueen campaign on legendary. you can get modules after each major encounter that drops a chest, but this can only be done once per character.
when events come around, like festival of the lost that will be in a couple of weeks, sometimes you will randomly get them from packages while doing the even activity.
You don't need to equip your highest-levelled items for loot progression, just having them in your inventory is enough.
Literally irrelevant.
As for OP: Your best bet for getting Upgrade Mods is buying them from Banshee, and trading planetary materials for glimmer with Rahool when needed.

The other materials you need (to purchase Upgrade Modules) you can get from dismantling things, and also from just doing Banshee-44's bounties. My recollection is that you can get Cores just by doing his bounties (although I could be mis-remembering).
There's also a mod that'll boost Core drop chances when doing activities (Strikes, Gambit, Crucible (mind you, there's one for each mode, so you have to switch)).
