Report a translation problem
1. Sound control panel - disable the "headset" option on whatever device your're using.
2. Turn on (or enable) Stereo Mix (not sure if that has anything to do with it, but I did it.)
3. Find App And Volume Preferences - make sure D2 AND STEAM has your headphones selected
4. In the Sound menu, under Input - Select Stereo Mix (again, not sure if this means anything, but mine is working now, so.)
5. in Sound Control Panel, under Playback> right click the device> properties> advanced> uncheck "allow applications to take exclusive..." hit apply.

How did you get the option to disable this?
Sound Settings > Device Properties (of whatever output device you use) > Additional Device Properties (its to the right, under related settings) > Advanced > Exclusive Mode

First an explanation - bluetooth headphones frequently include an in-line mic. For some strange reason if you use the "Headphones" device as your primary Sound output device instead of the "Headset" option, this issue occurs. The issue is that the "Headset" device option has crap audio quality.
Now that said, there's a very simple solution that doesn't require any of the above - tinkering and there's actually 2 options here.
Option 1. Disable in-game Voice Chat. Do this by going to Settings > Social > And simply toggling the "Voice Chat" option to off. Wait a second or two and voila - sound is back.
Now option 1 may not be feasible for those wanting to take advantage of in-game comms as opposed to using something like Discord, so there's an alternative. But if you don't intend to use in-game comms, option 1 is the simplest.
Option 2. Simply disable the "Headset" recording device in your system's Sound Properties. Do this by first pressing the Windows Key + R on your keyboard (Win + R). A run dialogue will appear. In that dialogue box, type "mmsys.cpl" without the quotes and hit enter. In this new window, select the "Recording" tab and locate your bluetooth "Headset." Ironically even bluetooth speakers with a microphone will be listed here as "headset." For the device you're currently using, simply right click it and select "Disable." The caveat to this is that you'll need to use a different microphone - ie.) USB, Analog, or a Cardioid Condenser Mic. In my case I've got a fairly nice Sterling Audio condense mic running through a pre-amp, so the bluetooth mic is simply not needed.
This allows me to use in-game voice chat, my bluetooth speaker or headphones, and/or use Discord/TeamSpeak/etc. at the same time.

Thank you very much! I know this is old, but the game is still broken lol. Tried every single other option to fix the issue with no luck (I use AT M50XBT), and finally the only fix was to turn off in-game voice chat and push-to-talk options.
