The first shot in the guns clip will do what ever the perk says. With,, I think a small cooldown. Very good on hand cannons to get a better chance at 3 hitting them with headshots in PvP. And also same with shotguns. The perk is basically useless in PvE

I wouldn't say useless. It's quite good on a Sniper if you wanna pick of that pesky hobgoblin
