Might be your skill level dude. Just an idea...

Could be their internet. Like, to them the other player is right in front of them but in actuality they're just a little to the side and it ends up causing a chunk of the gun's pellets to miss.
Nah, the client-side hit detection in D2 doesn't work like that. If you hit on your screen, you hit. Worst case scenario damage will be applied late due to latency.
I know hackusations are rampant in this game when it comes to relatively hard to aim guns like handcannons or snipers, which is understandable because those are things people are likely to aimbot with. But shotguns? at " a foot or two away"? I'd be surprised if you were serious. Those things live at point blank, which is within a few feet.
To OP. Probably not everyone cheating, most likely culprit is the internet connection. Either yours or theirs. Or as someone else mentioned you need to check the roll on your shotgun, smoothbore is one of the worst perks for a shotgun and will hurt you more than it will help.
Most of the time its horrible packet compression caused by people with stone age internet.
While there are also ample video proof of D2 hacks at work, OP, you have to admit, there is a possibility that you just came up against someone that much better at it.
Yup, some people out there are crazy good at the game. A lot of people watch people like that guy to learn from too, which kind of proliferates the crazy goodness. Given how forgiving D2 is in terms of aiming mechanics and how much practice some people put in, it's actually ridiculous what a good player with a good loadout can do in this game.
Hackers definitely do exist but they're nowhere near as common as a lot of people seem to think.
Aimbotting doesn't really work with a shotgun due RNG spread of your pellets (Your bullet).
