DX11 error out of memory

Hi there
my game keeps crashing in every session at random locations, this counts for single player as well as multiplayer.

I can predict it when it happens. GPU fan spin up wildly, game lags, freezes and crashes to desktop with an dx 11 error stating out of memory and a d3d error dxgi_error_device_removed in the second tab. bottom right states that my display driver has recovered

system:
i5 4460
16 gig ram
1T hardrive
asus GTX 580
windows 7 64bit

what i have done:
steam verify game
reinstalling dx10 and 11
reinstalling dotnet and C++
game setting as potato mode (everything as low or disabled)
reinstalled the display driver
set launcher and .exe to admin

i rolled back nvidia driver to v 337.88. seens to work for now. fingers crossed
